How can you qualify to Upgrade to the 2017 Wellness CDHP?

The State of Indiana is again offering a way to upgrade your health plan during Open Enrollment this fall.  The Wellness Consumer Driven Health Plan (CDHP) offers the lowest premiums compared to the other options. If you are currently enrolled in state medical benefits, you can qualify for the 2017 Wellness CDHP by reaching an Earned Status of Silver in HumanaVitality by August 31, 2016. This means all points must be processed and posted to your Humana Vitality account by the August 31 deadline. Don’t wait until the last minute to submit information as processing activities can take up to 45 days to be reflected in your account.

Use Humana ID to register for HumanaVitality

Employees covered by a state medical plan provided by the State Personnel Department should receive a welcome packet in the mail from HumanaVitality. Included in this welcome packet is a membership card that lists your unique HumanaVitality member number. You will be asked for this number when you register for the program.
