Step One is completing the Well-Being Assessment (WBA) at MyHealthyLifestyles.com.
Know Your Numbers
Step Two is completing a confidential biometric screening with your primary care physician or attending a free screening event in May, June or July.
Step Three is logging at least 200,000 steps in the Healthy Lifestyles Steps Challenge in July OR earning 2,500 points (in addition to WBA points) in MyHealthyLifestyles.com between June 1 and Aug. 31.
Are you ready to upgrade your health plan?
The State of Indiana is offering a way to upgrade your health plan during Open Enrollment 2015. To qualify for the Wellness CDHP plan, employees must currently be enrolled in state medical benefits, as well as complete three easy steps before Aug. 31, 2014. These steps help you to take control of your health and improve your overall well-being. By upgrading your health, you have the opportunity to upgrade your plan.

Step One is completing the Well-Being Assessment at MyHealthyLifestyles.com. This confidential assessment helps you to identify health risks, as well as giving you the opportunity to create a plan to address potential risks.COMPLETE YOUR WELL-BEING ASSESSMENT
Free Biometric Screenings available via IU Health
Step Two is completing a biometric screening with your primary care physician or attending one of many free screening events happening around the state in May, June and July. These numbers are reported to IU Health and kept confidential from your employer.

Complete your biometric screening with your primary care physician
You can also to have your doctor complete the Physician Option Form for State Employees and fax it according to instructions on the form. If your doctor has already completed these tests between January 1 and August 31, 2014, those results will be accepted on the form.
Health and Wellness Month is July
Step Three is completing the Healthy Lifestyles Steps Challenge in July or earning 2,500 points in MyHealthyLifestyles.com. To qualify for the Wellness CDHP, employees must log at least 200,000 steps during the month of the challenge. The 2,500 points earned in Healthy Lifestyles does not include the points earned for completing your WBA and the points must be earned between June 1 and Aug. 31.
The Upgrade program information applies to employees covered by a medical plan and employed by the State of Indiana by May 1, 2014, and does not apply to conservation officers, excise officers, Indiana State Police plan participants, part-time, temporary employees or contractors.
