ActiveHealth, a new voluntary wellness program for employees covered by a State of Indiana health plan, is a comprehensive tool that encourages lifestyle habits that reduce the risk of disease and promote good health. The ActiveHealth rewards program is one way it does that. It offers employees and covered spouses $100 e-gift cards for completing a biometric screening (they can also earn $50 e-gift cards for completing the ActiveHealth health assessment, see www.investinyourhealthindiana.com/activehealth for more on that).
A biometric screening is similar to a yearly wellness exam. The information gathered during this check includes: height, weight, BMI, waist circumference, blood pressure, blood glucose, and a full lipid panel (total cholesterol, HDL, LDL, and Triglycerides). You must follow certain restrictions before receiving a biometric screening.

Stay tuned for dates and more information about free state biometric screening events scheduled at state work locations. You can also complete your screening at your doctor’s office or a participating Quest Diagnostics Patient Service Center. To do so, choose one of the following three options:
- At a worksite screening – go to myactivehealth.com/stateofindiana and click on the Scheduling link.
- With your personal healthcare provider – using the Provider Results form.
- At a Quest lab – Note, the quest lab cannot use the Provider results form. You must go to myactivehealth.com/stateofindiana and click on the Quest link for instructions to use a Quest lab.
Below is a schedule of State sponsored Biometric Screenings. To schedule an appointment at one of these screenings, please visit ActiveHealth and click on the Worksite Biometric Scheduler link from the homepage. This will take you to the RepuCare site, where you will need to create an account. After you’ve created an account, you’ll be able to select from the list of biometric screening events. Closed sites are only open to employees who work at those sites. To schedule a screening at a closed site, please contact your HR team.

Important: Biometric screening results must be submitted by Oct. 31, 2019, in order to be accepted. To ensure this deadline is met, please schedule your screening at least two weeks prior to the date to allow ample time for the provider to process results.
