Premium Discount deadline is Sept. 30

With the premium discount deadline fast approaching, make sure you and your spouse (if applicable) finish strong and complete your chosen activity on time! Check the Rewards center in your ActiveHealth portal to see where you are on your wellness journey, and what you need to do to reach the finish line.

Remember: If you are completing the fitness tracking option, Sept. 30 is the last day to sync your tracker.

• Group Coaching: Complete four group coaching sessions in the same series through ActiveHealth.
• Health Coaching: Complete four individual health coaching sessions (in-person or virtually) through ActiveHealth.
• Health Education + Health Coaching: Complete two individual health coaching sessions (in-person or virtually) through ActiveHealth AND reach Level 3 through online education and challenges through the ActiveHealth Portal.
• Fitness Tracking: Meet the physical activity goal of 10,000 daily steps or 30 minutes of physical activity through a synced device on at least 200 separate days.

Complete a wellness visit for a $100 gift card

A wellness visit is an excellent way to get a snapshot of your current overall health. You’ll get results on key measurements such as BMI, glucose levels, blood pressure, and cholesterol. Complete your 2022 wellness visit for free by getting an annual physical from your primary care provider or by visiting a CVS Minute Clinic.

Complete a dental cleaning for a $50 gift card

Getting regular dental exams with cleanings can help find early warning signs of health problems and prevent dental problems. A dental exam with cleaning includes an exam by a dentist and scaling and polishing your teeth to remove any plaque and tartar build up.

Complete a health assessment for a $25 gift card

Log in to your ActiveHealth portal and complete a health assessment by Nov. 30 to receive a $25 gift card! It only takes about 10 minutes and your gift card is available to redeem immediately after completion in your ActiveHealth Rewards Center. Just hit “submit” and you’re good to go! Gift card rewards can be earned by employees and spouses on an Indiana State Personnel Department-sponsored health plan.
